I/we realize and acknowledge that participation in dance classes and activities could result in some possible personal injury including but not limited to: scrapes, bruises, cuts, sprains, fractures, broken bones, concussions, injury to the musculoskeletal and/or cardiorespiratory systems, which can result in serious injury or death, or injury or death due to a medical condition, whether known or unknown by me, or any personal property damage/loss, which may occur on the premises before, during or after classes. Despite precautions being taken by the studio, accidents and injuries may occur.By agreeing to this release form, I/we (the dancer and parent/guardian) assume all risks related to the use of any and all spaces used by GC Studios, LLC.   I/we agree to release from responsibility the GC Studios, LLC including all teachers, dancers, staff members, and facilities used by both entities from any cause of action, claims, or demands now and in the future. I/we will not hold GC Studios, LLC, liable for any personal injury including but not limited to: scrapes, bruises, cuts, sprains, fractures, broken bones, concussions, injury to the musculoskeletal and/or cardiorespiratory systems, which can result in serious injury or death, or injury or death due to a medical condition, whether known or unknown by me, or any personal property damage/loss, which may occur on the premises before, during or after classes. Furthermore, I/we agree to obey the class and facility rules and take full responsibility for my/our behavior in addition to any damage I/we may cause to the facilities utilized by GC Studios, LLC.I understand that GC Studios, LLC are licensed, accredited and insured organizations. In the event that I/we should observe any unsafe conduct or conditions before, during or after my/our classes, I/we agree to report the unsafe conduct or conditions to Gabriella Carter, owner, the administrator, instructors or staff members as soon as possible.

Tuition shall be billed monthly based on class rate and is payable via cash, check, zelle, or online through our automated payment system. Tuition will be automatically charged to the account on file on the 25th of each month for the following month's tuition. For all Cash, Paper Check or Zelle payments, there is a 3% Discount. Cash, check or Zelle payments are due BEFORE the 25th of the previous month. All families are required to have a current and working form of electronic payment on file in Parent Portal. Returned payments will be subject to a $30 fee; No Exceptions  Late payments will be subject to a $30 fee; No Exceptions. Installment rates are based on a full 40 week session; we do not prorate for short month, school/studio vacations, or holidays. Camps, Master classes, costumes, and special events are not part regular tuition and participation will be subject to additional fees.  No refunds will be given Classes may be made up within a 30 day period. Two installments will also be due upon enrollment, for the first and last month of classes. Withdrawal from a class (or all classes) requires 30 days written notice from parents. Families are responsible for tuition through the 30 day period.All dancers will pay a yearly membership fee upon enrolling in classes. We offer three different tiers of membership with aligning advantages; Basic, Premium, and Elite. Your membership fee charge will renew each year at the end of each season unless opted out. Families with more than one dancer will receive a 50% discount on their second dancer. I authorize GC Dance Company to automatically charge the monthly tuition and fees for my child(ren), using the payment method on file and I certify I am an authorized user of the account. If I wish to pay by cash or paper check, or from a different payment type than what is on file, payment must be submitted at least 1 day prior to the automatic scheduled payment being posted. If the date lands on a Sunday or a closed holiday, payment must be submitted before the studio is officially closed.

Students deciding not to continue with class/es at GC Dance Company MUST notify the office by filling out a cancellation form. Parents will be required to fill out a cancellation form which MUST be received by us no later than the 20th day of the month PRIOR to your last month of classes. Your last installment deposit will go towards your last month of classes, thereby releasing the parent from further tuition obligations. Parents will be responsible for the balance of tuition, if any, when discontinuing services. No exceptions will be made. This is the policy agreed to at the beginning of the season with enrollment. If you turn in the cancellation form after the 20th of the month, you will be required to pay tuition for the upcoming month. EXAMPLE: To withdraw from a class/classes on 12/31/23, written notice needs to be provided on/before 11/20/23.

PLEASE NOTE: We do not freeze memberships. By dropping we can not save your dancers spot or pause your monthly tuition. The Membership fee will be recharged if you decide to re-enroll midway through the season.

Installment payments are not refundable. Camp tuition payments are not refundable, once registered for camp. Event payments are not refundable.

Private lessons are separate from monthly tuition and each instructor has their own rate. Camps, competitions, costumes, master classes, special events, etc. are not included in monthly tuition and are additional fees. Missed classes can be made up within a 30 day period. Refunds or credits are not provided for missed classes. Class levels are not fully determined by a dancers age; our instructors can provide assessments, giving the appropriate level for each dancer. Classes can be removed from the schedule or altered, at any time. Classes need to have an enrollment of at least 5 dancers, to ensure it won't be removed from the schedule. If a class has a low attendance of 1 to 2 dancers one day (when typically this class has 5+ dancers enrolled), the instructor can choose to have a 30 minute class, instead of the scheduled length of the class.Cellphones need to be on silent. Ballet is required for: Level 2+ in Lyrical, Jazz and Contemporary. Ballet is not required for: Lower levels, Acro, Tap, Musical Theater or, Hip Hop.Tardiness: Dancers are unable to attend class if they are 15+ minutes late. Two recitals are held each year: Winter Showcase & Spring Showcase. Ballet Dancers level 1+ will be required to wear a GCDC leotard to class. ISPE paperwork will incur a yearly $75 fee. 

I give permission for my son/daughter's photographs/videos to be used for publicity and advertising in the studio, on the GC Studios, LLC website, and in the community.I hereby grant permission to the rights of my son/daughter's image, likeness and sound of my  son/daughter's voice as recorded on audio or video tape without payment or any other consideration. I understand that my son/daughter's image may be edited, copied, exhibited, published or distributed and waive the right to inspect or approve the finished product wherein my son/daughter's likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of my son/daughter's image or recording. By signing this release I understand this permission signifies that photographic or video recordings of my son/daughter may be electronically displayed via the Internet or in the public marketing or advertisement setting. I will be consulted about the use of the photographs or video recording for any purpose other than those listed above. There is no time limit on the validity of this release nor is there any geographic limitation on where these materials may be distributed. This release applies to photographic, audio or video recordings collected as part of the sessions listed on this document only. By signing this form I acknowledge that I have completely read and fully understand the above release and agree to be bound thereby. I hereby release any and all claims against any person or organization utilizing these materials.
